Job Description:

The Population Health Director will design and implement key Population Health priorities within an Oak Street division, working with various stakeholders including clinicians and care teams. This role is aimed at improving care quality, cost effectiveness, and driving innovation in health solutions.

Job Responsibility:

Own the execution and the ongoing refinement of Population Health programs in the local division
Work closely with functional leaders in the central office to assist with strategy and program design
Work side by side with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities
Accountability to key population health metrics, including quality measures, utilization measures, and financial measures
Other duties, as assigned

Requirements:

5+ years of applicable experience in healthcare / population health / working with healthcare providers, or other similar fields
Bachelor's degree required
Graduate degree preferred but not required (MBA, MPH)
Ability to build the trust and confidence of a wide range of stakeholders
Strong leadership and influencing skills
Incredible level of organization and detail orientation
Analytical and data savvy
Ability to develop business cases, ROI analysis, and financial analysis
Expert in Excel, Google Apps, project management
US work authorization
Pleasantly persistent approach
Quick learner comfortable with a high level of ambiguity
