Job Description:

Oak Street Health is on a mission to 'Rebuild healthcare as it should be', providing personalized primary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest. This role focuses on consulting, training, and leading Behavioral Health Specialists, with opportunities for leadership development and measurable impact on health outcomes.

Job Responsibility:

  • Training for new BHSs
  • Peer consultation to experienced BHSs
  • Participation in Cohort meetings
  • Conduct weekly cadence meetings with BHS providers
  • Collaborate with regional leadership
  • Advocate for BH program integration within clinics
  • Support BH providers to work towards achieving program metrics
  • Other duties, as assigned

Requirements:

  • Clinical license in the field of Mental Health required (LCSW, LCPC, LMTF)
  • Minimum 5 years post-Clinical experience in a Behavioral Health setting
  • Demonstrated advanced clinical skills in the duties, responsibilities, and skills as a Behavioral Health Specialist in a primary care setting
  • US work authorization
