Social Worker

• Conduct care management activities through telephonic and face-to-face interactions including member home visits
• Assess members’ health and social determinants of health needs
• Develop and monitor care plans and interventions
• Collaborate with providers, primary care physicians, pharmacists, and care management staff to track member progress
• Advocate for members to ensure they receive necessary care within their benefit plan
• Communicate with hospital social workers and community resources to manage care transitions and reduce preventable emergency room use
• Manage social work interventions such as psychosocial assessments

• Must possess active and unrestricted LMSW licensure in the state of Florida
• Minimum of 2+ years experience in medical social work or case management
• Must be willing and able to travel up to 75% of the time in the Miami Dade County area
• Reliable transportation required
• Must reside in or near Miami Dade county
• Master’s degree in Social Work is required
