Job Description:

The Center Medical Director will lead clinical teams to provide exceptional care for older adults, balance administrative responsibilities alongside clinical practice, and drive growth using a patient-centric approach.

Job Responsibility:

Support of organizational strategy in collaboration with Practice Manager and Outreach Manager
drive implementation of Oak Street Health's strategic initiatives
cascade organizational information to care team members and escalate concerns
coordinate and lead onboarding of new providers
model excellent medical care and success in metrics
ensure care teams effectively prepare for and execute daily tasks such as huddles, patient care planning, and admission prevention
educate and support providers in the value-based full risk care model
assess and address root causes for missed center metric goals
support and grow a positive and collaborative culture.

Requirements:

M.D., D.O., or A.P.N or P.A. graduates
Board certification in Internal Medicine or Family Medicine required for M.D. or D.O., Board certification in Family, Adult, or Gerontology for A.P.N.
Experience in clinical leadership roles, leading and coaching providers to be the best they can be for their patients and their colleagues
Experience using a metrics-driven approach to the provision of medical care and/or quality projects
Passionate about their work and motivated to be a part of a fast growing organization
Aware of their leadership skills and potential for impact and eager to share those with a fast-growing team
Commitment to older adults and to low-income patients
Comfort with ambiguity and a love for problem solving
Ability to remain calm in the midst of the storm and guide others to smart solutions
5+ years of experience in outpatient practice preferred
Fellowship training in Geriatrics, other professional degrees such as M.B.A., J.D., M.P.H. welcomed
US work authorization.
