Job Description:

The role of Center Medical Director at Oak Street Health involves leading a clinical team to deliver thoughtful primary care, implementing a value-based full risk model of care, coaching and mentoring providers, and driving exceptional patient outcomes.

Job Responsibility:

Work in collaboration with Practice Manager and Outreach Manager/Director to achieve organizational objectives
Drive implementation of organizational strategic initiatives
Coordinate and onboard new providers
Ensure clinical guidelines and care model implementation
Provide support and mentorship for providers
Conduct performance reviews and guide provider growth
Maintain culture of positive workplace and 'Oaky' values
Achieve center metric goals
Cascade organizational information and concerns between care teams and senior leadership

Requirements:

M.D., D.O., or A.P.N or P.A. graduates
Board certification in Internal Medicine or Family Medicine required for M.D. or D.O., Board certification in Family, Adult, or Gerontology for A.P.N.
Experience in clinical leadership roles, leading and coaching providers
Experience using a metrics-driven approach to medical care or quality projects
Commitment to older adults and low-income patients
US work authorization
5+ years of experience in outpatient practice preferred
Fellowship training in Geriatrics, other professional degrees (e.g., M.B.A., J.D., M.P.H.), and prior executive roles welcomed but not required

Nice to have:
