Job Description:

Develop, implement, support, and promote health service strategies, tactics, policies, and programs that drive the delivery of quality healthcare to our members. Health service strategies, policies, and programs are comprised of utilization management, quality management, network management, clinical coverage and policies. The position requires advanced clinical judgment and critical thinking skills in order to facilitate appropriate physical, behavioral health, psychosocial wrap around services.

Job Responsibility:

Conducting face to face visits using comprehensive assessments of members
Coordinate and collaborate care with member/authorized representative, PCP, and any other care team participant
Attend interdisciplinary meetings and advocate on members behalf
Develop care plan
Authorize services within the MLTSS benefit
Coordinate and assist with community resources
Document accurately and timely in the electronic health record
Use critical thinking and problem solving related to assigned membership

Requirements:

Must reside in Orleans, Jefferson, or St. Charles Parish
Willing and able to travel up to 75% travel within their region (no overnight travel)
Must possess one of the following active and unrestricted independent behavioral health licenses in LA: Licensed Clinical Social Workers (LCSW), Licensed Professional Counselors (LPC), Licensed Marriage and Family Therapists (LMFT), Licensed Psychologist
2+ years of behavioral health experience
1+ years’ proficiency with personal computer, keyboard, mouse, multi-system navigation
and MS Office Suite applications Outlook, Word, Excel, and Teams
Working hours will be Monday -Friday 8am-5pm. Potential on call rotating schedule for weekends, nights or holidays per business needs

Nice to have:
