The Clinical Informatics Specialist (Medical Scribe) supports primary care providers with clinical documentation in real-time during patient encounters. Responsibilities include observing visits in-person, documenting structured notes, assigning appropriate CPT and ICD-10 codes, and assisting with administrative tasks. The role involves on-the-job training in population health, chronic preventative care, value-based medicine, and team-based workflows. Job Responsibility:
Documenting patient encounters including joining the provider in the exam room, documenting structured notes, assigning CPT and ICD-10 codes, and preparing After Visit Summaries
consulting with the provider to ensure accurate documentation
requesting and reviewing medical records
leveraging population health tools for documentation improvement
preparing for and supporting Daily Huddles and Clinical Documentation Reviews
placing orders and referrals
addressing tasks and additional responsibilities related to clinical documentation
Requirements:
Knowledge of medical terminology and common medications, either from a pre-medical degree or prior clinical experience
prior clinical experience, including shadowing and/or volunteering
prior scribe or transcription experience
advanced listening and communication skills
strong computer literacy and ability to learn new technical workflows
fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve, where indicated
ability to adapt to new workflows and quickly learn new concepts and skills
ability to type 70+ words per minute
ability and willingness to take direction and be a member of a team providing patient care, including adapting to the provider's working style
excellent job attendance including ability to work in-person in clinics
ability to commit to at least 1 year in role, 2+ is ideal
ability to work approximately 40-45 hours per week during clinic hours with predictable hours and break times
compliance with hospital and Oak Street Health policies, including HIPAA
US work authorization

Prior scribe or transcription experience
fluency in additional languages such as Spanish, Polish, or Russian
