Job Responsibility:

Develop and implement comprehensive care plans tailored to individual needs
Conduct thorough and regular assessments, accurately monitoring progress and adapting plans as needed
Provide empathetic support and guidance to individuals and their families, fostering a collaborative approach to care
Effectively liaise with a wide range of healthcare professionals and agencies to ensure seamless care coordination
Maintain meticulous and accurate records and documentation, adhering to all relevant policies and procedures
Actively contribute to a positive and supportive team environment, sharing expertise and fostering collaborative working.

Requirements:

Valid NMC registration
Proven experience in mental health, ideally within a community setting
Excellent communication, interpersonal, and organisational skills are essential
Ability to work both independently and as part of a multidisciplinary team
A strong commitment to delivering high-quality, person-centred care.
