Job Description:

Join our dedicated team at the Pennine Care NHS Trust! We are seeking a compassionate Band 6 CPN Community Psychiatric Nurse to make a significant impact on individuals' lives. This is a fantastic opportunity to utilise your skills and experience within a supportive and collaborative environment.

Job Responsibility:

Develop and implement comprehensive care plans tailored to individual needs
Conduct thorough and regular assessments, accurately monitoring progress and adapting plans as needed
Provide empathetic support and guidance to individuals and their families, fostering a collaborative approach to care
Effectively liaise with a wide range of health care professionals and agencies to ensure seamless care coordination
Maintain meticulous and accurate records and documentation, adhering to all relevant policies and procedures
Actively contribute to a positive and supportive team environment, sharing expertise and fostering collaborative working

Requirements:

NMC registration
DBS on the update service
Proven experience in mental health, ideally within a Community setting ideally within the NHS
Excellent communication, interpersonal, and organisational skills are essential
Ability to work both independently and as part of a multidisciplinary team
A strong commitment to delivering high-quality, person-centred care
