Job Description:

The Field Case Manager RN is responsible for managing waiver members, providing direct face-to-face care, performing quarterly visits, and meeting contractual requirements. Key tasks include conducting comprehensive evaluations, identifying high-risk factors, ensuring high-quality care, and empowering members to make informed healthcare decisions. The role involves traveling 50-75% of the time within South Cook County. This position is critical to delivering enhanced, human-centric healthcare services.

Job Responsibility:

Conduct comprehensive evaluation of referred members' needs/eligibility
Facilitate appropriate healthcare outcomes for waiver members
Provide care coordination, support and education using care management tools
Coordinate and implement care plan activities and monitor progress
Identify high-risk factors and service needs that impact outcomes
Ensure maximum member engagement using motivational interviewing skills
Consult with case managers, supervisors, and Medical Directors to overcome care barriers
Utilize negotiation skills to secure services necessary for members' benefits
Help members participate actively in healthcare decision-making
Utilize case management processes in compliance with regulations and company policies

Requirements:

2+ years of clinical experience
Active and good standing Illinois RN license
Must possess reliable transportation and be willing and able to travel 50-75% of the time to meet members face to face in Southern Cook County, and surrounding areas

Nice to have:

Case Management and Discharge Planning experience
Managed Care experience
Microsoft Office experience
BSN degree
