Job Description:

Oak Street Health delivers high-quality primary care to Medicare patients through neighborhood centers and home-based care programs. This position supports logistics, clinical care, and documentation in our innovative home-based care program.

Job Responsibility:

Preparing visit schedule, route, and logistics for all visits
Accompanying nurse practitioner on all house calls
Vital signs, medication reconciliation, patient/family education
Documenting House Call note, managing referrals, orders and follow-up
Maintaining supplies/equipment
Other duties as assigned

Requirements:

At least one year experience as a Medical Assistant + MA certification
Must pass all clinical competencies before supporting patient care, phlebotomy a plus
Daily access to a vehicle with valid license and insurance
Flexibility to travel around Chicagoland
Bilingual (English and Spanish) a plus
US work authorization

Nice to have:

Bilingual (English and Spanish)
Phlebotomy experience
