Job Description:

The Patient Engagement Specialist will be part of the Population Health Team, conducting patient outreach via calls and home visits to foster engagement in a personalized care model. The role includes engaging with patients who have not accessed the clinic or care model, documenting interactions, communicating with care teams, educating about services, and traveling locally to support patient needs.

Job Responsibility:

Plan to spend about half of time in the community connecting with patients
Respond to escalations and urgent needs with prioritization skills and multitasking
Document interactions with patients in electronic medical records while maintaining HIPAA standards
Communicate complex topics between involved parties
Partner with patients and their support persons to identify goals and overcome barriers
Independently foster relationships with patients and their caregivers
Partner with Community Health Workers, Social Workers, and clinicians to support complex patient needs
Educate patients about Oak Street’s numerous wrap-around services

Requirements:

A track record of success in health-care-related engagement, sales, and/or marketing
Strong verbal and written communication skills
Strong technology skills and understanding of Excel as well as other EHR/EMR platforms
Dedication to serving the community and building meaningful relationships
A demonstrated ability to operate independently in a fast-paced environment
An understanding of the Medicare and Medicare Advantage programs
Access to reliable transportation and the ability to travel through assigned territories to connect with patients
Fluency in Spanish (written and oral) is preferred
US Work Authorization is required
