The RN Field Assessor is responsible for face to face assessing with members to evaluate the medical needs of the member to facilitate the member’s overall wellness and help them obtain the services they need to thrive. Addressing requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program and MLTSS program enrollment.
Job Responsibility:
Face to face assessing with members to evaluate the medical needs of the member to facilitate the member’s overall wellness and help them obtain the services they need to thrive
Addressing requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program and MLTSS program enrollment
Minimum of 10-15 assessments per week dependent on need, may be more
Using a holistic approach assess the need for a referral to clinical resources for assistance in determining functionality
Consults with supervisor and others on team in overcoming barriers in meeting goals and objectives
Utilizes case management processes in compliance with regulatory and company policies and procedures
Utilizes interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation
Mentor new hires on the team, once, a level of proficiency has been attained in their role
Requirements:
Must possess reliable transportation and be willing and able to travel up to 75% of the time within Mercer, or bordering Middlesex/Hunterdon County NJ
Must hold an active unrestricted Registered Nurse license in the state of New Jersey
2+ years clinical practice experience e.g., hospital setting, alternative care setting such as home health or ambulatory care required
Successful completion of the NJ Choice certification is a contingency of employment
Must be able to work Monday - Friday, 8:00am - 5:00pm EST

Assessment and Case management and/or discharge planning experience
Managed Care experience
Crisis intervention experience
NJ Choice certification
Clinical interviewing
Experience using personal computer, keyboard navigation, navigating multiple systems and applications
and using MS Office Suite applications (Teams, Outlook, Word, Excel, etc.)
