Job Description:

The RN Field Assessor is responsible for conducting assessments with members to evaluate their medical needs, facilitate wellness, and ensure necessary services are provided to help them thrive. The role involves extensive travel within Burlington and Ocean counties, while adhering to company policies and mentoring new hires.

Job Responsibility:

Perform face-to-face assessments with members to evaluate medical needs
Address requests for adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program, and MLTSS program enrollment
Conduct 10-15 assessments per week
Use holistic approach to assess need for referrals to clinical resources
Consult with supervisor and team members to overcome barriers in meeting goals
Utilize case management processes in compliance with regulatory and company policies
Mentor new hires

Requirements:

Must possess reliable transportation and be willing and able to travel up to 75% of the time within Burlington or Ocean County NJ
Must hold an active unrestricted Registered Nurse license in the state of New Jersey
2+ years clinical practice experience e.g., hospital setting, alternative care setting such as home health or ambulatory care required
Successful completion of the NJ Choice certification is a contingency of employment
Must be able to work Monday - Friday, 8:00am - 5:00pm EST

NJ Choice certification
Clinical interviewing
Experience using personal computer, keyboard navigation, navigating multiple systems and applications, and using MS Office Suite applications
