Job Description:

As a Social Worker in the Adult Services Team, you will work with adults requiring support, assessing their needs and providing person-centred care. Your role will involve conducting assessments under the Care Act 2014, developing personalised care plans, and facilitating access to services that promote independence, well-being, and long-term support. You will collaborate closely with service users, their families, and multidisciplinary teams to ensure their needs are met while adhering to statutory requirements and safeguarding procedures.

Job Responsibility:

Monitor, review, and maintain up-to-date support plans, ensuring they meet the changing needs of service users while delivering good value for money and advocating for their rights, choices, and access to services
Ensure that adults receive the appropriate support from the right professionals, tailored to their needs and promoting independence and well-being
Examine and oversee care strategies, ensuring that they are effective and sustainable
Support the assessment and care planning for high-cost placements and packages of care, ensuring appropriate resources are in place
Safeguard and promote the welfare of adults, adhering to relevant legislation and policies, including the Care Act 2014
Collaborate with service users, their families, and multidisciplinary teams to ensure seamless care and support
Maintain high standards of practice in line with statutory requirements and safeguarding procedures

Requirements:

2 years experience post qualification
Action safeguarding procedures
