Job Description:

As a Social Worker in the Older People and Disabilities Team, you will support adults with a range of complex needs, particularly older individuals and those with physical disabilities. Your role will involve assessing their needs, developing personalised care plans, and coordinating access to services that enhance their independence, well-being, and quality of life. This position focuses on providing person-centred care while ensuring adherence to statutory requirements under the Care Act 2014 and other relevant legislation.

Job Responsibility:

Completing annual Care Act Reviews for the residents of Hillingdon, which includes Older People, Adults with Physical Disabilities and Learning Disabilities
Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005
Make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate, ensuring legal compliance and protecting individuals' rights
Responsible for managing a caseload, conducting robust assessments in partnership with service users, their carers and multi-disciplinary partners where appropriate
Work closely with the Team Manager with a strengths based approach to provide personalised services with a clear focus on quality outcomes for our residents

Requirements:

Relevant social work qualification
2 experience in adult social work, including assessment and care planning
