Job Description:

An exciting Locum opportunity within a Mental Health Hospital Discharge Team in Stockport. The role involves supporting individuals experiencing mental health challenges and ensuring safe and timely hospital discharges.

Job Responsibility:

Part of a newly created all age adult mental health hospital discharge team
Work collaboratively with Pennine Care colleagues
Crucial part in improving mental health services and outcomes for people leaving hospital and their carers
Support older and working age adults with mental health diagnoses to be discharged from hospital safely and timely
Liaise with health, advocates, and third sector colleagues
Undertake mental health capacity assessments, chair best interest meetings, write reports, and attend tribunals

Requirements:

1-3 years experience of working within a similar social work setting
Diploma/Degree in Social Work
Registration with Social Work England
Experience of working within a mental health setting
